HOW IT WORKS
You fill out the assessment form with your health history, food preferences, and allergies. You will also write about your health goals.
Throughout this phone call, we will discuss your personal assessment and challenges that would prevent you from reaching your health goals. Then, we determine what would be best for you based on everything submitted in your personal assessment and your unique lifestyle needs (schedule, family, skills, values, etc)
Together based on your assessment and our conversation we devise a health and nutrition plan. Everyone is unique; therefore, your health and nutrition plan will be too. We will provide structure and guidance to help you reach your goals.
We will schedule follow up appointments to discuss how you’re doing on the plan, answer questions, set new goals, and make any adjustments if necessary. A follow up is about you getting the guidance and motivation you need to be successful
- Initial Coaching Session: 30-minute phone or video chat coaching session
- Custom Meal Plan: Based on your food preference and nutrition goals
- Grocery List: Complete list of groceries needed for your plan
- One time meal plan
- 1 Follow up coaching session: 30 minutes phone or video chat.
